Seabury Care Management Expands Service Area!
Seabury Care Management is expanding its service area with a new focus on Howard and Anne Arundel Counties in Maryland.
"We are excited to increase our presence in Maryland", commented Monica Micklos, Seabury Care Management Program Director. "The delivery of Covid-19 vaccinations and the loosening of pandemic restrictions will create increased demand for older adult services. Seabury is anticipating the need and investing our resources accordingly."
Seabury Care Managers serve clients through in-person visits and via video or telephone conferences. "The pandemic taught us that valuable support can be provided in video calls which helps expand our geographic reachā, Monica continued. āWe will serve Howard and Anne Arundel County residents in-person and virtually.ā
Care Management services are offered in three formats, tailored to the needs of clients and family members.
One-time Consultation: One-hour conversation via telephone or video conference with a licensed Care Manager. The Care Manager provides verbal feedback pertinent to the circumstances discussed. A one-time consultation can lead to further assistance either immediately or in the future.
Client Assessment: Client assessments are offered either in-person or virtually. An assessment requires several hours of research with the client and may include family members. The Care Manager produces a written report with specific recommendations addressing the assessmentās findings. Client assessments are offered at fixed rates for individuals or couples. An assessment can lead to further assistance either immediately or in the future.
Ongoing Care Management: Care Managers may be engaged in an ongoing relationship with a client/family. The scope of work and duration of the engagement is defined by the circumstances and expectations of the client/family. Ongoing Care Management services can be discontinued at any time by the client. Services can be terminated and reinstated by the client as circumstances dictate. An initial commitment of four hours is required.
