Interested in Working with Seabury's Care Management program?

Seabury Care Management is seeking Master level professionals in Social Work, Counseling or related field to provide care management to older adults in the DC Metro area.  Responsibilities include, ongoing monitoring and support through regular home visits with clients, identifying clients’ needs and linking to appropriate resources, mediating conflict, advocacy, and working with other systems, including families and medical and mental health teams.  Candidates must have keen assessment skills, excellent oral and written communication skills, possess strong organizational and problem solving skills, good record keeping habits and the ability to set priorities and meet goals. 

Documentation on computer necessary and car is required for home visits.  Appropriate licensure, malpractice insurance and car insurance required.  Membership with certification to the National Association of Professional Geriatric Care Managers is required for individuals whose profession does not offer professional licensure.  Experience in health care or geriatrics required. Bilingual in English and Spanish a plus. Contract positions available.
